چکیده

In the present study, we evaluated the renal response to a 4-hour infusion of amino acids in essential hypertensive patients, as well as the effects that dietary sodium restriction and enalapril (a converting enzyme inhibitor) had on this renal response. During normal sodium intake, amino acid infusion significantly increased renal plasma flow from 383 +/- 58 to 478 +/- 51 mL/min and glomerular filtration rate from 82 +/- 8 to 100 +/- 13 mL/min. All these effects were abolished when the patients received a low sodium diet (40 mmol/d) for 3 days before the amino acid infusion. The administration of enalapril to the patients during sodium restriction restored the amino acid-induced increment in renal plasma flow (from 388 +/- 35 to 573 +/- 48 mL/min) and glomerular filtration rate (from 88 +/- 9 to 103 +/- 10 mL/min). Mean arterial pressure remained unaltered under all experimental conditions. The results show that in patients with essential hypertension dietary sodium restriction prevents amino acid-induced increments in glomerular filtration rate and renal plasma flow and that this effect is restored during the simultaneous administration of enalapril.

Attilio Losito, MD, Servizio di Nefrologia e Clinica Medica, Policlinico, I-06100 Perugia (Italy) Dear Sir, An association between glomerular hyperperfusion and proteinuria has been established in spontaneous hypertension in rats, a disease with many characteristics similar to those seen in essential hypertension (EH) in man [1]. We have recently found that untreated EH patients, with apparentl...

Effects of amino acid infusion on renal hemodynamics. Role of endothelium-derived relaxing factor.

Ingestion of protein or intravenous infusion of amino acids acutely elevates glomerular filtration rate (GFR) and renal plasma flow (RPF) by unknown mechanisms. Endothelium-derived relaxing factor (EDRF), now known to be nitric oxide derived from metabolism of L-arginine, participates in local regulation of vascular tone. To investigate the hypothesis that EDRF may participate in the renal vaso...

Early alteration in glomerular reserve in humans at genetic risk of essential hypertension: mechanisms and consequences.

Essential hypertension has a familial predisposition, but the phenotype of elevated blood pressure has delayed penetrance. Because the kidney is a crucial determinant of blood pressure homeostasis, we studied early glomerular alterations in still-normotensive young subjects at genetic risk of hypertension. Thirty-nine normotensive adults (mean age 29 to 31 years), stratified by genetic risk (pa...

Stimulation of the kallikrein-kinin system by pregnancy may help restore renal vasodilator response to glycine in Dahl salt-sensitive rats.

To the Editor We would like to contribute to the discussion of possible mechanisms involved in the pregnancy-induced restoration of the renal vasodilator response to glycine in Dahl salt-sensitive rats reported by Whitescarver and Heesch in the December issue of Hypertension. We want to call attention to the role of renal kinins, postulated as mediators of amino acid-induced hyperperfusion and ...

Renal effects of amino acid infusions in patients with panhypopituitarism.

Strong evidence indicates that a high protein diet accelerates end-stage renal disease by increasing glomerular capillary pressure subsequent to renal vasodilatation. The mechanisms underlying this vasodilatation remain undefined, but they have been suspected to be mediated by a pituitary factor. To test this possibility, we measured changes in renal plasma flow and glomerular filtration rate i...
